You are requested to read these very carefully before preparing your
manuscript. This will ensure optimal reproduction of your paper.

In order to ensure the widest dissemination of your article with
appropriate copyright protection, please sign and return this form.

Offprint Order Form [alt.

If you wish to order offprints, please return the completed order
form with the quantity clearly identified. Note that offprints can
only be ordered in multiples of 50 and are available 4-6 weeks after
publication.
- Colour Confirmation Form [alt.

The cost for colour reproduction in the printed version is €500 for
the first page and €250 for the following pages, excluding V.A.T.
Authors' agreement of the prices must be clearly indicated by
completing and signing the attached Colour Confirmation Form
otherwise illustrations will be produced in black and white. It will
not be possible to change figures to black & white or colour at the
proof stage.
Figures supplied in colour that appear in black & white in the
printed version of this volume will now be available IN COLOUR,
online, on ScienceDirect.
